The C5 dermatome is supplied sensation by the C5 nerve root. This
nerve exits the middle part of the neck at the C4-C5 disc. Patients
usually complain of shoulder weakness and having difficulty raising
the arm over the head. |

The C4-C5 level is in the middle part of the
neck as shown.

When the neck joint (facet) connected to
this level gets irritated, it refers pain to the lower neck and
top of the shoulder.
